Intérêt de la téléréhabilitation dans la prise en charge du patient atteint de bronchopneumopathie chronique obstructive

Abstract : Background: Chronic obstructive pulmonary disease (COPD) is a chronic inflammatory lung disease which is a major public health problem. Only a minority of COPD patients have access to pulmonary rehabilitation (PR). Home-based solutions, such as telerehabilitation (TR), have been used in order to make PR more accessible. Aims: The goal of this systematic review was to investigate the effects of TR on physical capacity and dyspnea in patients with COPD. Methods: The Randomized Controlled Trials (RCTs) were identified into three medical databases : Pubmed, PEDro, and Cochrane Library. Study outcomes had to include measures of at least one of the following key outcomes : 6-minute walk test (TM6) distance, shuttle walk test (ISWT) distance, dyspnea, dyspnea-related quality of life, impact of COPD symptoms on daily quality of life, anxiety and depression. Results: Five RCTs (460 patients) were included. The results showed that TR significantly increased the distance covered in the TM6. The study also showed that, in the ESWT, TR improved dyspnea, quality of life, anxiety and depression. Indeed, the results found were similar to those of a center-based respiratory rehabilitation. Conclusion: The use of TR may lead to improvements in physical activity level, although the results must be interpreted with precaution given that the heterogeneity of the studies. This is an important area of research and further studies on the effect of this recent therapy, would be beneficial to get a better study and conclude to significant results
