Allaitement douloureux et soin ostéopathique : protocole d'étude pour un essai clinique randomisé contrôlé

Abstract : Introduction: breastfeeding is recognized as the best food for newborns and is recommended by all major medical authorities. However, its implementation remains sometimes difficult and one of the main reasons for stopping prematurely is nipple pain. At present, while many studies are beginning to support the different mechanisms of action of osteopathy as well as its wide field of applications, there are still very few studies on the management of painful breastfeeding. However, two preliminary studies carried out in the maternity ward of Bordeaux University Hospital suggest its effectiveness in continuing breastfeeding. Aims: in the first part, we present the study protocol for a randomized clinical trial without blinding. Its primary aim is to compare the usual management of painful breastfeeding with or without osteopathic care for the mother-newborn dyad, with respect to the rate of breastfeeding at one month after birth. 80 dyads characterized by a nipple pain greater than 7/10 will be recruited and randomized into two groups: an “intervention” group which will receive, in addition to the routine care, an osteopathic treatment, and a “control" group. In a second part, we present as preliminary results, a sample of the population already benefiting from osteopathic care in the maternity ward of Bordeaux University Hospital. Conclusion: if it shows efficiency, osteopathy care can help many women to succeed in their breastfeeding wish and improve the percentage of breastfeeding women.
