Prise en charge urologique des complications du port prolongé des anneaux péniens : étude rétrospective nationale française et revue de la littérature

Abstract : The "penis captivus" is a nosological entity described since the 12th century. Different clinical presentations are possible due to a wide choice of external genital strangulation devices and their treatments are varied. The objective was to review the current state of the art in France of the management of penile ring complications and to compare it to the literature. Material and method: this french multicenter retrospective study is based on a collection of data from clinical cases, their treatments and their sequelae, carried out using an online questionnaire sent to urologists, members of the Andrology and Sexual Medicine Committee of the French Urology Association between June and October 2020. We also conducted a literature review. Results: sixteen urologists responded, including 14 who had already been confronted with the case, once or several times. Of the 37 patients analyzed, 70% wore an exclusive penile ring. In 22% of the cases, the ring was handcrafted. Three patients had serious complications. Six others had acute urine retention. Ring removal was manual in 9 cases, per section in 23 cases and surgery was required in 5 cases. Analysis and discussion: despite a limited number of responses to the questionnaire, however, we have a high number of cases compared to the literature. It appears that complications are more important when the delay before the consultation is extended or the device is tightened. There is no age range of patients more affected but the reason for use varies from decade to decade. Conclusion: knowing how to manage this type of complication implies being quick, ingenious about withdrawal and vigilant about the immediate and late consequences, particularly in the management of genito-sphincter disorders, as well as knowing how to detect possible psychiatric symptoms or look for abuse. This type of complication, even rare, also exists for vacuum. It is therefore important to educate patients when it is prescribed.
