Ouverture de la prescription des traitements antiviraux à action directe de l'hépatite C : intérêt porté par les médecins généralistes d'Ille-et-Vilaine et identification des freins potentiels à la prescription

Abstract : Background: Following the development of direct-acting antivirals (DAAs) treatment for hepatitis C, the World Health Organisation (WHO) has set the objective to eliminate viral hepatitis as a public health problem by 2030. In France, to target a larger population, the prescription of DAAs is allowed to general practitioners (GPs) since March 2019. Objective: define the proportion of GPs in Ille-et-Vilaine who accept to be trained to administrate this new prescription. Method: Quantitative observatory study. An online questionnaire has been sent to the GPs working in Ille-et-Vilaine. We analysed 117 responses. Results: GPs monitor an average of 2.3 patients with chronic hepatitis C. 67.5% (n=79/117) of the doctors studied were not aware of the possibility to prescribe DAAs. None of the physicians have been prescribing these treatments. Only 32.5% (n=38/117) of the GPs were interested to be involved in delivering DAAs. However, the majority of the doctors studied 86% (n=101/117) accepted to be trained. Discussion: The main limits were the inexperience of GPs and the low prevalence of HCV in their patient population. The interpretation of tests for hepatic fibroses was also an obstacle. Conclusion: A wider communication towards GPs about DAAs seems necessary. GPs are interested in receiving training on DAAs and non-invasive measurement of hepatic fibrosis.
