Épidémiologie de l’AVC à Mayotte : incidence, mortalité, caractéristiques et étiologies

Abstract : Background : We would like to complete the knowledge of stoke epidemiology in Mayotte. The objectives of the study are to calculate the incidence and mortality of stroke and analyse risks factors and causes in this particular population. Method : The collection of datas is retrospective. We Screened informatics files coded stroke between 2013 and 2017at the hospital of Mayotte. Were included every adult victim of stroke on the land of Mayotte and admit to the hospital in the very beginning of symptoms. Results : 553 patients were selected including 463 with ischemic stroke. The standardized incidence was 159,9/100.000 people. The one of ischemics strokes was 125,9/100.000. The acute mortality was 20,5/100.000. 78% were ischemic stroke including 13% of cardioembolic, 15% due to large vessel, 42% due to small vessel disease ans 30% of undetermined cause. The mean year occurring stroke was 62 years for men and 63 years for women. 48% of patients had less than 65 ans. Hypertension prevalence was 88,5% and only 57% under treatment. Patient with social protection had better chance of being treated for his hypertension. Hypertension was more prevalent on lacunar and microvascular strokes. The prevalence of diabetes was 33% with 9% of mean HbA1c. Diabetes complication were more présent on patient without social protection. Conclusion : The population of Mayotte appears more exposed to neurovascular lesion and particularly microvascular damages. Mostly due to the important prevalence of hypertension and diabetes
