The present thesis exhaustively explores the central role of the family in the care of patients at risk of
psychotic transition (CHR-P) in psychiatry. This research has focused, through a comprehensive literature
analysis, on three crucial domains: the role of the family as a risk and protective factor, specialized
services dedicated to CHR-P individuals, and existing family therapies with a particular emphasis on their
outcomes. This study has unveiled how the family can influence the risk of psychotic transition in CHR-P. It
has also delved into specialized services tailored for CHR-P and examined the gradual integration of the
family therein, including recommendations, organizational models, and types of CHR-P services. Various
family therapeutic approaches currently available for the care of CHR-P individuals, such as Family
Focused Therapy (FFT-CHR), Enhanced Care (EC), psychoeducational multifamily groups, among others,
have been reviewed, shedding light on their outcomes.
This thesis provides a holistic perspective on family involvement in the care of patients at high risk of
psychotic transition. It contributes to the understanding of family involvement in the treatment of high-risk
patients for psychotic transition, highlighting significant advancements in this field while identifying
persistent challenges to be addressed.
La présente thèse explore de manière exhaustive le rôle central de la famille dans la prise en charge des patients à risque de transition psychotique (CHR-P) en psychiatrie. Cette recherche s’est concentrée via une analyse de la littérature sur trois domaines cruciaux : la place de la famille en tant que facteur de risque et de protection, les services spécialisés dédiés aux CHR-P, et les thérapies familiales existantes avec un accent sur leurs résultats. Cette étude a révélé comment la famille peut influencer le risque de transition psychotique chez les CHR-P. L’intégration progressive des familles au sein des services spécialisés a été étudiée. Notamment les recommandations, les modèles d'organisation et les types de services CHR-P. Les différentes approches thérapeutiques familiales actuellement disponibles pour la prise en soins des CHR-P, telles que la Famille Focused Therapy (FFT-CHR), les soins améliorés (Enhanced Cared EC), les groupes multifamiliaux psychoéducatifs, entre autres, ont été passé en revue. Cette thèse offre une perspective holistique de l'implication de la famille dans la prise en charge des patients à haut risque de transition psychotique. Ce travail contribue à la compréhension de l'implication familiale dans le traitement des patients à haut risque de transition psychotique, mettant en lumière les avancées significatives dans ce domaine tout en identifiant les défis persistants à relever.
