La grossesse chez les adolescentes

Abstract : Objectives : to assess the specific difficulties in the follow-up care for pregnant teenagers, from an obstetrical and pediatric point of view, taking social considerations into account also.

Material and method : from January 1st 2006 to December 31st 2008, 64 teenagers gave birth in the teaching hospital of Angers. We retrospectively studied the physical characteristics, the socio-economic situations of these young women and their partner, their medical history, the progression of their pregnancy and the specific follow-up put in place, the delivery mode, the new-born features, the evolution in the post-partum, and finally the social conditions of the discharge from the maternity ward.

Results : all patients were between 15 and 17. The majority were single and living with their parents. One young woman out of three had a significant social history (e.g. Fostering, mistreatment, rape). We observed more high-risk behaviours (addictions, anorexia nervosa) than in the same age general population. The fathers were between 15 and 41. 76% of the pregnancies were regularly followed-up, always within the framework of the “PASS-Périnatalité” consultations. There was a clear increase in the risk of the prematurity (15,5% versus 5% in the general population). The labour was physiologic in 71,9% cases. The mean weight of newborns (3010 grams) was slightly lower than the national medium (3300 grams). We did not observe serious complications during post-partum, neither for the mother nor for the child. A social follow-up was planned in 76,6%, with the young mothers' agreement.

Conclusion : these patients present a high-risk of premature childbirth. It is needed to establish a fitted medical, psychologic and social follow-up.
