Les sujets âgés de plus de 65 ans se présentant aux urgences de Chambéry après une chute‎ : données de l'évaluation initiale au regard des recommandations HAS : devenir des patients à 1 et 3 mois de la sortie

Abstract : It is common for older patients to present to accident and emergency departments after a fall. Up to now, few studies have focused on the care process of these people in emergency departments. This study aims to observe the emergency department management for elderly who consult after a fall and to compare it to the most recent HAS recommendations of April 2009. This descriptive and retrospective study included 166 patients over 65 years who have successively consulted for falls at the emergency department in Chambery hospital during 4 weeks, from November 16th to December 13th 2009. The outcomes of the study come from the main objectives of the consultation contained in the recommendations of the HAS 2009, namely: examination of walking, testing of postural hypotension, electrocardiogram, physiotherapist referral. A recall of patients directly discharged from emergency department was then performed at 1 and 3 months. The study data shows an incomplete implementation of the HAS recommendations. The management remain focused on the injury sustained. Patients discharged directly from the emergency department did not recover well, especially during the first month. We observed 21.8% of readmissions and 15.6% falls in this month. These results underline the need for an early assessment of patients in the month following the fall. The emergency department has an important role in identifying individuals at high risk of recurrence and functional decline. Finally, a local protocol would probably be interesting to facilitate compliance with the HAS recommendations and improve care for seniors making repeated falls.
