Facteurs de risque de détresse respiratoire chez les prématurés modérés (32 à 34 semaines d'aménorrhée)

Abstract : The respiratory distress syndrome (RDS) is an important cause of neonatal morbidity and death. It is less frequent in infants born at 32 to 36 weeks of gestation (wog) but late-preterm infants (34 - 36 wog) and intermediate-preterm infants (32 - 34 wog) represent the majority of the preterm births. The purpose of this study was to look for respiratory distress risk factors in a population of babies born at 32 to 34 wog. A total of 579 new-borns were included in the study, 107 had a RDS. Pathology of the placenta (placenta praevia and abruptio placentae), neonatal infection, primary caesarean section, low gestational age, male sex, low Apgar scores at one and five minutes and intubation in the delivery room were associated with the occurence of RDS. Multiple pregnancies, preterm labour and early premature rupture of the membranes lasting more than 48 hours seemed to reduce the risk of respiratory distress. When logistic regression was performed, low gestational age, male sex, pathology of the placenta and neonatal infection, were associated with an increased risk of RDS (pathology of the placenta: odds ratio, 2.90; 95% CI, 1.34-6.24 and neonatal infection: odds ratio, 20; 95% CI, 4.11-97.41). Early premature rupture of the membranes was associated with a decreased risk (odds ratio, 0.27; 95% CI, 0.10-0.75). The increased risk of RDS in infants of mothers with placenta praevia with or without primary caesarean section has already been described. The physiopathology is still unknown. The foetal inflammatory syndrome associated with early premature rupture of the membranes leads to enhanced lung maturation and could explain the decreased risk of RDS.
