Optimiser la prescription médicale du sujet âgé en soins primaires‎ : impact de l'utilisation de l'outil STOPP (Screening Tool of Older Person's Prescription) sur les prescriptions médicamenteuses inappropriées

Abstract : Introduction: The STOPP criteria, a PMI screening tool, has been used mostly in hospitals, whereas it seems to be an effective tool to review the requirements of the elderly by general practitioners in primary care. Our study aims at evaluating if the use of STOPP, during prescription renewal performed by general practitioners, could lead to a decrease of PMI. Method: 20 GPs have been involved in this study while using the tool STOPP for 10 consecutive patients. These patients had to be over 75 years and to take each day at least five different drugs. The MAI score assessing the quality of prescribing, was used for statistical comparison of orders before and after intervention. Results: The use of STOPP has reduced the number of PMI according to STOPP for nearly half of the patients (46.9%) and the MAI score of the PMI of 39.1% (p<0.001). The improvement in MAI according to STOPP is mainly related to the number of medication regardless of age prescribers (≥ 50 years). The use of STOPP also allowed an improvement of all prescriptions (the global MAI decreased by 14.3%, p <0.001). Conclusion: This is the first study showing a reduction of PMI with STOPP criteria in primary care. This improvement mainly concerns the list of PMI STOPP but also the overall quality of its requirements. It seems necessary to confirm these results by a randomized study; it would be lawful to promote the use of STOPP, during the synthesis annual consultations (ALD) or even for the prescription renewal consultation so as to reduce PMI.
