Identification des freins des médecins généralistes à pratiquer le dépistage des violences conjugales auprès de leurs patientes‎ : étude qualitative par entretiens semi dirigés avec des médecins libéraux et salariés en Isère

Abstract : BACKGROUND: Domestic violence against women is a public health problem, which is not enough spotted by general practitioners. Our main objective is to identify barriers to screening. It is also to identify the representation doctors have of their role, and to know better what they need to improve their practice. METHOD: Qualitative study with 17 semi-directive interviews conducted with general practitioners, either liberal or employed in Health Centers or in Planning Familial. RESULTS: It is recognized that intimate partner violence screening is the responsibility of general practitioners. Their main obstacle to the treatment of victims is a lack of awareness: ignorance of the prevalence, of certain risk situations (perinatal period) and of potential severity of the impact (death hardly mentioned). The other barriers are material (lack of time, lack of availability), emotional (feeling of failure related to the complexity of the situation, desire of self protection) and logistical (lack of tools, lack of training, unfamiliarity with the network). Systematic screening is badly accepted. Family doctor has a particular awkward position. Salaried doctors feel more comfortable with the problem than liberal ones. CONCLUSION: Raising awareness of doctors and public, developing and promoting a network are solutions to improve intimate partner violence screening.
