L'immunothérapie spécifique au venin d'hyménoptère‎ : étude comparative de 2 protocoles, à 100 et 150 microgrammes

Abstract : The treatment against hymenoptera venom allergy is specific immunotherapy, consisting of repeated injections of a maintenance dose of allergen, in order to promote tolerance. The aim of this study was to compare two different maintenance doses, 100 and 150µg, with evaluation of rate changes IGES and of threshold of positive skin tests, and tolerance of the maintenance doses and of re-stings.This retrospective multicenter study used the médical records of patients who completed at least three years of treatment. These patients were desensitized to the 100μg dose in Grenoble and 150μg in Montpellier. In each medical record, different follow-up data were collected, including the rate of IGES and skin test results.On 179 collected files, 117 patients had been re stung, with one grade III reaction, and three grade II reactions. The 100μg maintenance doses were well tolerated in 98,9% of cases, versus 92,05% with the 150μg maintenance dose. On five years follow-up, decrease of 82,58% of the bee venom median rate was observed in Grenoble, versus 98,98% in Montpellier (p=0,09). An improvement of the bee venom skin tests was observed in 65,12% et 66,67% of cases, in Grenoble and Montpellier. These datas were 86,11% and 75% when the yellow jacket venom was used, in case of a desensitization to this venom. From the differences we observed in tolerance, efficacy, skin and biological tests' improvement, our study provides the use of the 150μg bee venom maintenance dose is more useful, and the 100μg maintenance dose used in the yellow jacket venom allergics. These results remains pertinent even regarding in increased costs.
