Représentations et vécu de la grande précarité par le personnel des urgences du centre hospitalier de Chambéry

Abstract : Providing care to patients in situation of great poverty in an Emergency Department (ED) is often described as a difficult task. The purpose of this study is to better understand the Chambéry Emergency staff's experience working with such patients and their representations. We analyzed 34 semi-structured interviews of various categories of workers in Chambéry ED in this qualitative study. Their representations show how complex and diverse great poverty is. The homeless, illegal foreigners and isolated elderly people are mentioned. These people suffer from a lack of shelter, financial resources, relatives or relations, and therefore have difficulties in developing and maintaining good health. For persons in need, ED is the most common entrance to health care. ED workers' experience many difficulties in dealing with them: often resulting in feelings of weariness and care-failure because of repeated consults, as well as difficulties to establish a care relationship due to aggressiveness or refusal to receive care. Few workers have received a subject-specific training for this. Most of them wish to learn more about the social aide networks available for this population outside the hospital. Improving the spotting of these "at risk" people, promoting the information available to medical staff about available resources both in and outside of the Hospital , and developing and systematizing the help of social workers may allow the situation to progress. This would promote better care management of these patients and would make caring for these patients easier for the medical emergency staff.
