Imagerie et classification des modalités de division de la veine porte intra-hépatique

Abstract : We present an original anatomic study of the intrahepatic portal three, based on a 307 abdominal MSCT analysis. Portal vein division variations are frequent (23,45%) and easily recognizable on 3D MIP views. Their detection is imperative before an interventional procedure (portal vein embolization, TIPS creation) or a complex surgical procedure (partial liver resection, liver transplantation). This classification provides a guide to the operating technique and detect the contraindications: type 1 is favorable for all interventions; type 2 is less favorable: it doesn't represent a contraindication to interventional radiology procedure but it makes the surgical split of the liver more difficult; type 3 need changes in interventional and surgical procedures because ignoring it could lead to major complications; type 4 contraindicates any interventional technique on the liver.
