Quelle place pour l'utilisation des traitements nicotiniques de substitution pendant la grossesse en médecine générale ?

Abstract : Background: Maternal smoking is a major public health issue. Its adverse effects on maternal and fetal health are well known. The use of nicotine replacement therapy is recommended by health authorities. The objective of this review is to determine the effectiveness and the safety of using this treatment during pregnancy. Methodology: The literature review focused on international publications with high level of evidence published between 1997 and 2012. The effectiveness of nicotine replacement therapy was judged on smoking abstinence. Their safety was deemed on perinatal morbidity and mortality. Results: None of the trials published to date has demonstrated the effectiveness of nicotine replacement therapy during pregnancy. The animal studies suggest that nicotine administrated during pregnancy is teratogenic. The trials in humans show maternal and fetal changes without obvious clinical consequences. There are no studies evaluating the long-term toxicity of prenatal exposure to nicotine in humans. Conclusion: The effectiveness or safety of nicotine replacement therapy during pregnancy are uncertain at this time. Further studies are needed in this regard, taking into account problems that have faces existing studies.
