Dépistage de l'anévrisme de l'aorte abdominale‎ : taux de dépistage dans une population de médecine générale et faisabilité par des infirmières

Abstract : INTRODUCTION: Abdominal aortic aneurysm (AAA) diagnosis is clinically difficult and consequences of aneurysm rupture are fatal. The echography is the best way for AAA screening, it is simple and it reduces the mortality rate. OBJECTIVE: The objective is to evaluate the AAA screening rate in a family practice population and the feasibility of this screening by trained nurses. METHOD: The screening rate was studied on 448 patients met in 4 family practice offices in Savoie and Haute-Savoie on two periods of on month (April and July 2012). The reproducibility of the nurses was studied on a hospital population of 109 persons from October 17th to December 15th of 2011 in Annecy. RESULTS: Only 55.5 % of the patients recommended by the French society of vascular medicine were detected. No patient was specifically sent for this screening to a specialist. The reproducibility of the nurses versus expert doctor is good (kappa = 0.734). For a size of the aorta lower than 3 cm, there is 98.4 % of matching results between the doctor and the nurses. CONCLUSION: This study confirms a low rate of screening in the concerned population. The nursing screening could be a useful and effective alternative to reduce the mortality of AAA. It would also allow to rationalize medical time without abandoning screening.
