Utilisation du Syntocinon® au cours du travail spontané : évaluation des pratiques professionnelles à la maternité de Port-Royal

Abstract : The use of the Syntocinon® during spontaneous labor: evaluation of the professional practices at the Maternity of Port-Royal. Despite its frequent use, the Syntocinon can cause abnormal fetal heart rate, hyper stimulations and postpartum hemorrhage. However, there is no French recommendation on its use during spontaneous labor. The Maternity of Port-Royal established a protocol aimed to supervise the use of the Syntocinon. When this protocol is not respected, do the undesirable effects are more important? To answer that question we have realized an audit of the professional practices at Port-Royal. All the primiparous patients at the term, in spontaneous labor and who received Syntocinon during their labor in 2011 have been included (N=475). The duration of landings of increase of the Syntocinon turned out to be the least respected criterion. When it was inferior to the twenty minutes recommended, this duration was associated to an increase of the post-partum hemorrhage, including severe bleedings and Apgar scores at 5 minutes ≤7 (all p<0,05). Sensitize professionals to the respect of the protocol seems to be necessary to improve maternal and neonatal outcomes.
