Accompagnement des patients diabétiques au cours du jeûne du Ramadan‎ : implication et besoins des équipes officinales

Abstract : Ramadan can have an impact on the care of diabetic patients. The food rythm changes, alternating complete fast during the day and feasting at night. This rythm's impact on the glycaemic balance varies a lot. The risk for complications increases during the day. Recommandations allow to identify each patient's level of risk, and thus an adaptated strategy can be proposed. This work's aim is to analyse the pharmacy teams' position : actions proposed and potential needs. A qualitative exploratory survey has been conceived. All 27 professionnals involved in this survey consider that Ramadan fast is risky, and even sometimes contra-indicated. In that case, 63% of them don't try to convince the patient not to fast. 63% of the pharmacy team members offer specific care for diabetes : raising glycaemic controls, advice to avoid hypoglycaemia, sometimes medication schedule adaptations. 48% of the pharmacy team members think they can get enough information from the patient to provide them with a set of actions, but 81% feel they haven't been trained enough. This survey depicts the delicate situations emerging from discussions about religious practice between patients and carers. Pharmacy team members often rely on other professionnals to convince the patient not to fast. A pluri-professionnal care seems indispensable. The pharmacist could organise a meeting between doctors, nurses, dieteticians, and Imams. The skilled pharmacists, within a care team, could, counting on the strategic position of the pharmacy, optimise diabetic patients' care not only during Ramadan, but also in the long term.
