Ciblage thérapeutique du microenvironnement tumoral : exemple du cancer du pancréas

Abstract : Pancreatic cancer currently remains one of the most aggressive cancers, with a life expectancy of 5 years still lower than 5 % and a median survival of six months after diagnosis. In addition, incidence is increasing, and the number of cases is expected to double from now to 2030. The dramatic mortality associated with this cancer is caused by the difficulty in the early diagnosis, by the extreme aggressiveness of the cancer, and by the ineffectiveness of current treatments. It has been widely demonstrated that the active microenvironment of pancreatic ductal adenocarcinoma, the most common cancers of the pancreas, plays a major role in the quick proliferation and survival of cancer cells, as well as their propensity to metastasize. It is also implicated in drug resistance of pancreatic tumors. This microenvironment is characterized by a strong fibrous desmoplastic reaction which consists of a very dense extracellular matrix, associated with a cell component (fibroblasts, endothelial and inflammatory cells). Close and complex communication between cancer cells and the stromal cells tends to make cancer cells the most suitable to tumor development. The development of new therapies against pancreatic adenocarcinoma is an essential axis of research, and targeting the microenvironment appears as a solution more and more considered and explored in recent years. It has already shown very promising results in various preclinical and clinical studies, and should allow the development of more effective treatments for pancreatic cancer.
