Icatibant, Firazyr® : indications actuelles et utilisations potentielles

Abstract : The icatibant, developed under the name HOE-140 and marketed under the brand name Firazyr®, has been the first B2 receptor antagonist (B2R) to obtain a marketing authorization. The B2R is the target of bradykinine (BK), a vasodilator peptide produced after the contact phase activation. This peptide is involved in the regulation of the vasodilatation and the endothelial permeability with responsibilities in inflammatory phenomena, blood pressure regulation and edema formation. At the moment, icatibant possesses as unique indication the rare disease management of the hereditary angioedème with C1 inhibitor deficiency. Nevertheless its use was proposed in other pathologies (ischaemia reperfusion, chronic inflammatory pathologies as asthma, leak syndrome) without clinical trials demonstrated its efficiency. To understand why icatibant efficiency was not shown in these clinical trials and to envisage other applications it is important to apprehend the interactions between BK, icatibant and B2R in the physiopathology of the vascular permeability.
