Facteurs prédictifs de césarienne en cours de travail chez la primipare à bas risque. Étude comparative rétrospective cas-témoins à l'hôpital Jean Verdier de Bondy

Abstract : Predictive factors for caesarean section during labor in nulliparous low risk women. In nulliparous women, one out of five births is a cesarean section. We tried to identify predictive factors for cesarean section during labour within a low risk population. Our retrospective comparative case-control study included 255 patients. A more frequent and early use of synthetic oxytocin, abnormalities of the fetal heart rate occurring before 4cm of dilatation, and epidural placement when the fetal head is high while cervix dilatation is under 3cm are associated with risks of cesarean section during labour (all p<0,05). These results, related to obstetric practices, encourage a less active management of labor. Research opportunities concerning latent phase, active phase and stagnation of cervical dilatation will allow to less medicalize the first stage of labor.
