Mesure des lactates au scalp fœtal au cours du travail : analyse des pratiques professionnelles dans une maternité de niveau III

Abstract : Our main objective was to observe the practice of the fetal blood sample (Lactate) during the labor in our Establishment. We wanted to know if the pH could be substituted by the lactate. For that, we have seen two characteristics of those techniques: the difficulty of those prelevements and the capacity to diagnosis a fetal acidosis during the labor. Sixty lactate and pH prelevements has been collected simultaneous. The second part of study consisted in making an audit to the Basse-Normandie maternity. To evaluate the practice in each establishments. The results of the study shows both techniques was similar (R=0.945). Lactate had a better sensibility to predict a pHao < 7,20 (40% vs 27%) and better in the prediction of low apgar ( < 4 at 5min: 25% vs 0%). The proportion of pH failure was higher (p < 0.001). The result of the audit shows that the level 1 maternity didn't have any second line techniques. Lactates are easier than pH to perform, with a similar capacity to predict fetal acidosis. Lactates are less expensive than pH and more accessible for the little maternity. The medico legal problem who stopped many maternity to use this, will disappear with the income of the new equipment connect with a print system.
