Réalisation effective et tolérance des mesures barrières (port d’un masque et lavage des mains) par les patients pris en charge par leur médecin généraliste pour un syndrome grippal

Abstract : Effectiveness of vaccine against seasonal influenza for populations at risk remains subject to debate. The annual population coverage is decreasing and risk of an influenza pandemic is real. Barriers measures, which are low-cost and simple, must keep a key role in the real influenza epidemics. The effective implementation of these measures has not been studied in primary care in France. We conducted a pilot intervention study in the Paris region in early 2013, on patients with ILI, who were proposed by their general practitioner to put into practice the wearing of surgical masks and hand washing. Among 90 patients included by 11 general practitioners, 68 have responded to the questionnaire. 71% of respondents reported having worn the mask at least once with an average daily duration of 4:40 hrs, the main drawback being the ridiculous outlook (54%). 84% of patients who did not wear the mask had already an influenza related disease against 56 % of patients who wore a mask (p<0.05). Of the respondents, 60% washed their hands at least 10 times a day. Patients with a general practitioner working in sector I or working in a group practice or tutor or females reported better implemented hand hygiene (p=0.01, 0.01, 0.03, 0.04). Patients reported having properly put into pratice barriers measures were more convinced of having influenza (p=0.10). Patients who never had flu had a more serious representation (p<0.05) of it. Patients living with fragile subjects did not apply more barrier measures. This study shows a good reported implementation of barrier measures in primary care in an influenza epidemic. The general practitioner has a key role to play in educating the patient.
