Caractéristiques et prévisions à cinq ans de l’offre de soins primaires des 5e et 6e arrondissements parisiens : enquête transversale auprès des médecins généralistes, gynécologues et pédiatres

Abstract : Objective: We investigated the characteristics of primary care in the 5th and 6th arrondissements of Paris in 2013 and its evolution within 5 years. Method: As part of the study DEMOMED, we sent a questionnaire between June and October 2013 to all general practitioners (GPs), gynecologists and pediatricians with a liberal activity, hereby assessing sociodemographic characteristics, the organization of their cabinet, the mode of exercise, the access to care, the volume of activity, the practitioners’ experiences and the evolution of their activity within 5 years, drawing thus the portrait of the predictable primary care in 2018. Primary endpoint: The primary endpoint was the 5-year project, "departure", meaning cessation or decrease in activity over 25%, versus "continuity" as defined by stable or increasing activity over 25%. Results: 49 GPs, 7 gynecologists and 5 pediatricians responded (respectively 36%, 13% and 33%). By 2018, 44% of the GPs (divided into 23% departure and 21% decrease), 29% of the gynecologists and 20% of the pediatricians forecast a cessation or a decrease of their activity. For the GPs, the significant differences between the groups “departure” and “continuity” are age and gynecological activity. Various trends are emerging for 2018: computerization of records, less consultation without appointment, increased clinical activity per practitioner, reduction of house calls and generalization of lower consultation pricing. Conclusion: Our study shows an alarming decline of primary care within 5 years in the 5th and 6th arrondissement, making it urgent to rethink its organization.
