Étude descriptive d’une population d’adolescents obèses de grade 2 à l’île de La Réunion : détermination du statut en vitamine D et de l’association des concentrations sériques en 25-hydroxyvitamine D avec les composantes du syndrome métabolique

Abstract : Objective: Due to potential effects on prevention of cardiometabolic diseases, we defined the vitamin D status of a population of obese adolescents to Reunion Island and we determined the association of serum 25-hydroxyvitamin D [25(OH)D] concentrations with the different components of the metabolic syndrome, the insulin sensitivity indexes (HOMA-IR and QUICKI) and the inflammation state. Methods: Ninety eight obese adolescents aged 9 to 18 years were recruited over a nine month period. The components of metabolic syndrome and the HOMA-IR and QUICKI indexes were analyzed in relation to 25(OH)D concentrations after adjustment for age, sex and BMI by the Pearson correlation coefficient before applying an analytical model of multiple linear regression. Results: The mean concentration of 25(OH)D was 28.4 ng / ml. The prevalence of the deficit was 5.5% and that of the insufficiency of 49.5%. Nearly 22% of adolescents had a metabolic syndrome. The concentrations of 25(OH)D levels were correlated with waist circumference (r = -0698, p <0.001), systolic blood pressure (r = -0244, p = 0.017), HDL-cholesterol (r = 0.286, p = 0.005) and triglycerides (β = 2.115, p = 0.037) levels, HOMA-IR (β = -0.343, p = 0.001) and QUICKI (β = 0.339, p = 0.001) indexes. No association could be revealed with glycaemia and glycated hemoglobin levels. Vitamin D deficiency may contribute to the installation of low-grade inflammation accompanying obesity (p <0.001). Our data suggest a participation of the daily intake of breakfast to improve the inflammatory state and vitamin D status. Conclusion: Vitamin D may play a preventive role in the development of cardiometabolic diseases in obese adolescents. Confirmation of these results would lead to the establishment of promotional actions for a systematic vitamin supplementation in this population in primary care.
