Accidents des guides de haute montagne français de 2003 a 2013 : étude rétrospective de 286 cas et de leurs conséquences

Abstract : Context: The exposure to danger is inherent to the job of mountain guides. Objective: The objective was, after having described them, to evaluate the consequences of the accidents of the French mountain guides between 2003 and 2013. Method: It was a retrospective epidemiological study of the accidents reported to the national union named Syndicat National des Guides de Montagne. The physical, psychological, social, financial consequences as well as the quality of life by the score WHOQOL-Bref were evaluated through an evaluation form. Results: 286 accidents with 70 casualties (mortality rate of 4,35 per year for 1000 guides).Those guides were younger than the average. Mountaineering accidents (34%) occurred during the climbing with roped up guides, followed by accidents in off-track skiing (18%) or country skiing (13%). The human and environmental weaknesses were equivalent, the first one being the fall (39%), then the avalanche (23.4 %). The lower limb represented 41% of the injuries, then the trunk (24%), the upper limb (21%) and finally the head (14%). 22% of the accidents were moderate or serious (9% had a NISS >15, 13% between 9 and 15, and 78% <9). As a physical consequence, 50% had after effects and 22% had to reduce their working time. The quality of life of the mountain guide was superior to the average in France as far as the whole WHOQOL-Bref is concerned. A post traumatic stress disorder was frequent (8% complete disorder, 14% sub-syndromal disorder).The accident had family (33%), social (16%) and financial (57%) consequences. The social insurances were not sufficient or showed many lacks. Conclusion: Being a mountain guide exposes to frequent and very serious accidents with many consequences.
