Les émotions du médecin généraliste dans la prise en charge de la fin de vie : modalités d’expression et stratégies de gestion : étude qualitative auprès de 10 médecins généralistes du département du Lot-et-Garonne

Abstract : Introduction : Owing to the unique relationship that a general practitioner can create with a patient and his circle, he must face his own emotions and does not often have a chance to express his feelings. The main objective of this study is to record GP’s feelings during end-of-life care. Methods : A qualitative research including 10 GPs of the Lot-et-Garonne departement was conducted from January 2014 to March 2015. Semi-structured interviews were analyzed by thematic analyses. Results : This study highlights the fact that end-of-life care is very emotionally charged. The place of emotions that can be felt is ambivalent. Some of them think emotions must be part of the care, others, that their feelings must be contained. Emotion’s influence can be seen as profitable. When the feelings are seen as a damage, GPs refuse to let themselves be guided by their emotions. Sharing and taking away feelings are the two principal ways a GP can use to deal with his emotions. They are conscious or an unconscious process. What is holding them up to share is the fact that finding a proper listener is not easy. As well as mentioning their intimacy with other care givers. Many organizational and emotional difficulties are mentioned, and also strategies to face up to them. Few of GPs had listening experiences during their studies. They recognize the interest to be trained to express feelings during internship. They suggest the development of talking group and role-playing group. Conclusion : This work allowed to give GPs a chance to speak about their feelings during palliative care, to kwow their strategies of coping and to start a reflection on the need to be trained to express one’s feeling.
