L’accompagnement global en maison de naissance : étude descriptive des issues maternelles et néonatales

Abstract : A birthing center is a structure managed by midwives, offering global support to women with a low-risk pregnancy. In France, its trial is allowed since November 2013. We have conducted a descriptive and retrospective study on maternal and neonatal outcomes of women who intended to give birth at birthing center (in the CALM, "association for birthing center of Bluets") between 2013 and 2014. Two groups were defined aiming to work with a homogenous population: women who have been followed and accepted for starting labor the CALM (n = 105); and women who could not enroll in CALM (for non-medical criteria) and who gave birth in a conventional maternity (n = 20). A review of records and interviews were conducted. The goal of our study has been to describe the maternal and neonatal outcomes of these two groups and to assess the satisfaction of women from conventional maternity group. At CALM, women mainly used non-medical analgesia (96,5%), mainly give birth by spontaneous vaginal birth (89,5%). 66,7% of women require suturing of perineal lesions. The rate of maternal transfer was about 24,8% and the one of neonatal transfer was about 1,9%. Multiparous have more favorable outcomes and are less moved than primiparous. In conventional maternity, women have resorted epidural analgesia in 65,0% cases, mainly give birth by spontaneous vaginal birth (75,0%). 66,7% of women require suturing perineal lesions. They are satisfied with their care. Newborns have good adaptation to extra-uterine life in the two groups. This study doesn’t compare our two groups but describes them. Other comparative, prospective studies looking at larger scale would be needed.
