Évaluation du risque d’embolie gazeuse durant les transplantations pulmonaires

Abstract : Lung transplantation is the curatif treatment for terminal respiratory failure. Air embolism is one of its potential complications. As it is,this surgery implies an anastomosis of the pulmonary graft on the left atrium. The incomplete purge of the graft can consequently make one vulnerable to a systemic embolism. To our knowledge, no study has yet taken a cautious look at air embolism during lung transplantation. The main objective of this retrospective study is to evaluate the incidence of air embolism during lung transplantations that took place between 2011 and 2014 in the Foch Hospital Transplant Group. The secondary objective is to determine the intraoperative results on bispectral index, electrocardiogram and hemodynamics parameters. All of the patients with mono or bipulmonary lung transplantation were included within this research. The primary aim was to assess the amount of air from the left ventricule and aorta with transesophageal echocardiography. A hundred and eighty three patients were transplanted during the period previously mentionned. The age of the patients ranged from 16 years old up to 60 years old. Etiologies that related to transplantation were cystic fibrosis, pulmonary emphysem and pulmonary fibrosis. Incidence of air embolism was 76 %. A majority of patients (70 %) presented stage 1 or 2 of air embolism. Stage 3 of air embolism appeared to have been quite uncommon. In 323 purges the bispectral index dropped 12 times. Stage 2 and 3 of air embolism were associated with a drop of the Bispectral Index value. The results of this study encouraged us to take a closer look at neurological or myocardial dysfonction in postoperative by conducting another specific research that would ultimately enable us to acquire a much better understanding of the subject.
