Facteurs prédictifs de sevrage et de survie à 30 jours d'une assistance par ECMO artério-veineuse, dans les suites d'un infarctus du myocarde : analyse d'une cohorte rétrospective

Abstract : Purpose: ECLS provide efficiency systemic circulatory in case of acute myocardial infarctions complicated with acute cardiac failure. However the mortality related to this technics remain. The aim of this study is to assess predictive weaning and survival at 30 days factors in patients under ECLS for AMI. Materials and Methods: patients were retrospectively included from prospective database including all ELCS supported in the Grenoble University hospital. Demographic, anamnestic, hemodynamic and biological data’s before ECLS and 3 days after were recorded. Results: 117 patients were included between January 2008 and July 2014. The main ECLS indication was cardiogenic shock for 60 patients follow by refractory cardiac arrest. A successful percutaneous revascularization was performed for 87 patients. In univariate analysis, low-flow duration, arterial lactates before ECLS and assistance duration was significantly higher between survivor at 30 days and non-survivors (p<0.001) as well as weaning and un-weaning patients (respectively, p<0.001, p=0.006 and p=0.004). Multivariate analysis established that arterial lactates was an independent predictive factor of 30 days mortality (OR:1.8 ; IC95%[1.0021-1.3835]. Conclusion: Arterial lactate rate before ECLS initation could be a accurate method to select wich patients may benifits of ECLS use.
