Évaluation de la tomographie par émission de positons au 18F-fluorodéoxyglucose couplée à la tomodensitométrie dans le bilan d’extension, l’évaluation thérapeutique, le bilan de récidive et la surveillance des sarcomes utérins

Abstract : Purpose : To assess the usefulness of 18F-FDG PET/CT for staging, restaging, evaluation of recurrence, treatment monitoring and post-therapy surveillance of uterine sarcomas. Methods : A retrospective multicentric study was performed between December 2003 and December 2013 on patients who underwent 18F-FDG-PET/CT scans for uterine sarcomas. Only patients with follow-up data were included in the study. A positive lesion on 18F-FDG- PET/CT was defined as a focal abnormality detected on PET, without regards of the CT component. Results : Out of 61 patients who underwent a PET/CT scan during the study time, 45 patients 16 were excluded, 45 were studied (23 leiomyosarcomas, 11 stromal sarcomas, 11 carcinosarcomas) and underwent with 114 18F-FDG-PET/CT. 18F-FDG-PET/CT had a sensitivity of 85%, specificity of 97%, positive predictive value of 99%, negative predictive value of 74% and accuracy of 89%. These values were lower for surveillance (sensitivity between 50 and 75%) and higher for leiomyosarcomas than the other types. 18F-subtypes. FDG-PET/CTC did not show prognostic value. Conclusion : 18F-FDG-PET/CT may be a useful tool for staging, restaging, evaluation of treatment and monitoring of recurrence in uterine sarcomas, especially leiomyosarcomas. Its interest seems lower for surveillance. The absence of prognostic value may be due to small sample size of the cohort and need to be confirmed.
