Grossesse, VIH et secret médical : regards sur des professionnels de la périnatalité face à un dilemme éthique

Abstract : INTRODUCTION: The HIV epidemic rise to a controversy surrounding the intangible nature of medical confidentiality. The establishment of an effective screening strategy in early pregnancy can diagnose a third of HIV-positive women. Thus, the treatment of patients infected by perinatal professionals is sometimes complicated by the non-disclosure of HIV status to partners and exposing caregivers to the dilemma confronting the ethical principle of non-assistance to persons in danger against absolute obligation medical confidentiality. STUDY OBJECTIVES: To identify the position and explore the feelings of professionals deal with this ethical dilemma. To identify the information provided to the patient as well as the communication strategies used to get him to reveal the secret. MATERIALS AND METHOD: This is a retrospective descriptive qualitative study of perinatal care professionals who have ever participated in their professional career in the management of HIV-infected woman during perinatal period and refusing to inform their spouse of his health. Data were collected from 7 interviews according to the method of comprehensive interview. Content analysis was then performed. RESULTS: The study showed the existence of various professional positions more or less clear, given the ethical and legal issues raised by the bounden duty of medical confidentiality. Faced with the dilemma, the emotional experiences of the participants revealed a multiplicity of feelings, emotions and the main difficulties associated with the different opinions of everyone. Finally, the survey revealed the diversity of communication strategies and guidance used by professionals, from the fortification of the relationship of trust with the use of the concept of risk CONCLUSION: This study confirms the existence of ethical and legal issues arising from the absolute obligation to confidentiality. The diversity of selected issues and personal opinions reflects the complexity of the dilemma. To search a single solution would not be appropriate; it is for the perinatal professionals to develop a solid medical, psychological and social support.
