Insuffisance de renseignements cliniques sur les prescriptions de biologie médicale : points de vue croisés des prescripteurs et des biologistes

Abstract : Introduction: A preliminary study conducted around the town of Marmande showed a lack of clinical information on laboratory test requests, both in ambulatory and hospital care. In light of these results, this study was focused on the prescriptions made by the general practitioners. This study was aimed at finding the reasons for the lack of information, from the viewpoint of both general practitioners and clinical biologists. Methods: A qualitative study was conducted between June and July 2015, using semi-structured individual interviews with general practitioners and clinical biologists, in the departments of Gironde and Lot-et-Garonne. The recruitment was based on a snowball sampling method and inclusions were stopped when data saturation was achieved. Results: Data saturation was obtained after the interview of 10 general practitioners and 8 clinical biologists. Clinical biologists explained the lack of clinical information by time shortage, insufficient training and insufficient knowledge of the benefits of it. On the other side, general practitioners point out the feeling that doctors can interpret the results themselves, the habits and the feeling that information is not necessary for the clinical biologist to assess the results. Conclusion: General practitioners don’t respect the statutory obligation to indicate clinical information on laboratory test requests. Many reasons are declared but the main one seems to be the lack of communication between the prescriber and the clinical biologist. Through this study, it appears to all participants that the best way to restore the communication between the two groups is by using the prescription and the test results report.
