Impacts de la chirurgie sur la sexualité des patientes souffrant d'endométriose pelvienne profonde sévère

Abstract : Introduction: The deeply infiltrating endometriosis (DIE) affects sexual life of women. The principal aim of this study is to assess the impact on sexuality of the surgery, at 6 month post-operative. Materials and Methods: Prospective bicentric study done on Bordeaux and Toulouse hospitals. Patients who underwent surgery for symptomatic DIE between November 2013 and November 2014, and compliant with inclusion criteria, have been included. Each patient has answered to the multidimensional sexuality questionnaire validated in French, the “Brief Index of Sexual Functioning for Women” (BISF-W) which includes a composite score (CS). Pain symptoms have been evaluated with Visual Analogy Scale (VAS). Questionnaires were answered before surgery, 3 months after (for VAS), and 6 months post-operative. An analysis in subgroup depending on GnRH analogues (GnRHas) has been realised. Results: 20 patients have been followed up to 6 months post-operative. In preoperative, global sexuality was significantly altered with regards to a reference population (CS=21.8±8.4 versus 32.2±12.6, p=0.001) and particularly affected arousal, intercourse frequency and pleasure. At 6 months, global sexuality (CS=21.8±8.6 versus 30.5±7.6, p=0.002) and major part of questionnaires domains have been improved. Global sexuality has become comparable to our reference population (CS=30.5±7.6 versus 32.2±12.6, p=0.573). A significant improvement of dyspareunia, non-cyclic pelvic pain and digestive pain has been shown at 6 months. Subgroups analysis, depending on GnRHas treatment, did not show differences on sexuality. Conclusion: Deeply infiltrating endometriosis surgery improves sexual life of patients and their pain symptoms, at 6 months postoperative.
