Place de la chirurgie bariatrique par sleeve gastrectomie chez le patient bipolaire obèse : nouvelles perspectives

Abstract : Obesity has a direct adverse effect on both the somatic and the psychiatric course of bipolar disorder, likely through the low grade inflammation associated to obesity. Since weight loss improves significantly the inflammatory profile, treating the bipolar obese subject goes hand in hand with an appropriate care of the excess weight. Bipolar patients seem to achieve a weight loss comparable to others after bariatric surgery without an increased risk of a psychiatric decompensation, and yet, many are denied surgery as psychiatrists choose the road to precautionary management. Our objective is to assess the benefit/risk ratio of the laparoscopic sleeve gastrectomy in bipolar obese subjects through our case studies and an extensive literature review. In the case studies, follow-up data at 1 year showed, on average, a 32.8% total body weight loss among bipolar obese patients who underwent sleeve gastrectomy, a result comparable to the general bariatric population. Somatic outcome was favorable, no psychiatric hospitalization was reported, and quality of life and depression scores were improved. As our results match the recent literature review, a favorable benefit/risk ratio of a sleeve gastrectomy in this group of patients is expected. Those preliminary encouraging results could make psychiatrists less wary of sleeve gastrectomy and promote future studies with larger samples to confirm these findings. A recommendation algorithm as well as a study protocol to address a number of unanswered questions are suggested at the end of the thesis.
