Techniques de chirurgie plastique appliquées au cancer du sein

Abstract : Introduction: When breast conservative treatment (BCT) is performed for breast cancer, oncoplastic surgery (OPS) allows a wider excision and in sano surgical margins even in the inner and lower quadrants or for larger tumors. The first objective of this study is to compare surgical margins between different oncoplastic approaches. The second objective is to compare surgical margins and recurrence-free survival between standard BCT and OPS for cancer located in the outer quadrants of the breast. Methods: This is a retrospective study conducted on women treated for breast cancer in Curie Institute between 1998 and 2013. Women treated by OPS (lateral mammoplasty, batwing, Inverted T or nipple-areolar-complex excision) for in situ carcinoma (n=145) or invasive carcinoma (n=817) where included as well as those treated with lumpectomy for a breast cancer located in the outter quadrants (n=4371). Results: The rates of positive margins and of second surgery (re-excision or mastectomy) are respectively 21.1% and 14.2% for invasive carcinoma and 34.3% and 25.6% for in situ carcinoma. There was no difference when comparing the different OPS techniques. For invasive carcinomas of the outer breast quadrants, the rate of positive margins was significantly higher in case of lateral mammaplasty compared to lumpectomy (p<0,01) but the groups were different. In contrast, disease-free survival and global survival were not different at 5 years. Conclusion: OPS is reliable for the surgical treatment of breast carcinomas. Women should be informed of the risk of completion mastectomy in case of positive margins. Long-term local recurrence is still to be determined.
