Intérêt de l'ECMO veino-veineuse dans le syndrome thoracique aigu chez les patients drépanocytaires

Abstract : Background: End stage renal disease is a main concern in French Caribbean territories. A higher proportion of urgent-start dialysis is found in these territories compared to mainland France. Our aims were to describe the characteristics of patients at initiation of dialysis and to determine factors associated with urgent-start dialysis. We also estimate quality of life at 3 month after first dialysis and overall survival at one year. Material and methods: We performed a descriptive and prognostic study between January 2014 and June 2015 in Guadeloupe and French Guiana. Descriptive analyses were performed and data were compared according to urgent-start or planned dialysis. SF-36 general quality of life and specific KDQoL questionnaires were recorded. Kaplan Meier analyses were performed to estimate overall survival. Results: We included 242 patients (184 in Guadeloupe and 58 in French Guiana). An urgent-start dialysis was found in 112 (46.5%) patients. The multivariate analysis showed that risk of urgent-start was associated with nephrologist visits during the year preceding the first dialysis, with arteriovenous fistulas before dialysis and with cardiovascular comorbidities. Quality of life at 3 month did not differ between groups and was not associated with overall survival. The dimensions of quality of life were higher in our population than in studies conducted in dialyzed patients in mainland France. Conclusion: This first study shows that main factors associated with urgent-start dialyses are the lack of nephrology follow-up and preparation for blood dialysis in end-stage renal disease patients.
