La remédiation cognitive dans l’anorexie mentale de l’enfant et de l’adolescent, une nouvelle perspective thérapeutique ?

Abstract : Among all kinds of mental disorders, anorexia nervosa is the pathology with the highest death rate. The results of the usual therapeutics regarding this disorder are not satisfactory, so we must come up with new approaches. Cognitive remediation is an expanding therapeutic modality in the field of psychiatry. It is still in development phase in the field of anorexia nervosa. For the aforementioned pathology, this therapeutic tool is based on the results of the recent works examining the neurocognitive functioning of these patients. These studies have outlined cognitive particularities such as a lack of central coherence and of mental flexibility which are precipitating factors which then maintain the disorder. During this research, we questioned whether cognitive remediation could be regarded as a therapeutic lever in the field of anorexia nervosa and, specifically, among the pediatric population. To find answers, we studied the literature regarding this subject. This research first provides an overview of the characteristics of anorexia nervosa among children and adolescents. It then presents the modalities and principles of cognitive remediation, as well as its main applications in psychiatry. Finally, it concentrates its application in the field of anorexia nervosa, focusing specifically on studies conducted among children and adolescents. There are still few studies and most of them are case studies. These studies are confronted with methodological difficulties, in particular regarding the choice of a good measurement tool allowing to perform a specific assessment of the effects of cognitive remediation, as well as of the ‘’good’’ control condition. As a matter of fact, the current studies do not provide any conclusive evidence regarding the improvement of cognitive performances post CRT. An improvement of symptomatology and of the overall functioning of the patient that is underlined following the CRT cannot be attributed solely to CRT considering the assessment methods applied in the existing studies. Nevertheless, this literature review made it possible to highlight the value of this therapy, first of all as it creates a quality therapeutic alliance allowing a durable therapeutic engagement in the pediatric population. These promising results need more in-depth cover through new studies, in particular through the implementation of randomized controlled studies.
