Dysfonctions rénales aiguës après transplantation pulmonaire sous circulation extracorporelle : étude observationnelle sur 6 ans

Abstract : Introduction : acute kidney injury (AKI) is a common adverse event after lung transplantation (LTx). Postoperative AKI increases morbidity and mortality. The aim of our study was to describe the factors associated with AKI after LTx, related to donor, recipient characteristics and to the perioperative phase. Methods : We performed a retrospective one-center, observational, retrospective, cohort study of all adult patients who underwent LTx at the University Hospital of Bordeaux between 2010 and 2015. LTx were realized according to the SEQUENCEC protocol. AKI was defined according to the RIFLE criteria (Risk, Injury, Failure, Loss, End stage) in the first two days after LTx. Comparative analysis was performed between the two groups with or without AKI. Results : one hundred and ten underwent LTx during this period. AKI occurred in 70 cases (64%) within the first two days after LTx. Cardiopulmonary bypass was used in 98% of cases, with 70% of them with the SEQUENCEC protocol. Perioperative amount of transfused packed red blood cells and norepinephrine dose were associated factors with AKI after LTx. The 30-days mortality was 7%.The 30-days Mortality for all lung transplanted patients in this period was 9,5%. Conclusion : risk factors in our study are related to the perioperative phase. They may be prevented or avoided and we have to consider them to define prevention strategies and improve perioperative management of lung-transplanted patients.
