Doit-on intégrer les « médecines alternatives » dans les systèmes de santé ? Éléments d'analyse générale, cas de la recherche clinique

Abstract : "Alternative and complementary medicines" (CAM) are riding high to the point where institutions like the World Health Organization (WHO) would States incorporate them into their health care system. Such a project this is called "integrative medicine". Is such a project ethically justifiable ? The identification and analysis of WHO's argumentation enabled us to highlight their weakness and vagueness and the relevance to carry out clarifications and analyses of the concepts of "CAM", "medicine" and "integrative medicine". This work enabled us to divide our issue into many sub-questions. Among these sub-questions we choosed the issue of ethical justifications to begin clinical trials to assess MAC's therapeutic effectiveness. We support the idea that there are no conclusive arguments for or against and that the decision to perform clinical trials for a specific MAC is a political choice. In view of such a choice thirteen criteria were put forward and illustrated by a case study of the MAC "osteopathy in the cranial field". This case analysis enabled us to shed light on the challenges that would face the potential promoter for a clinical trial in osteopathy in the cranial field. More generally our work revealed that there is no ethical justification to spreading integrative medicine if not clarified by field of application (clinical practice, clinical research, etc.).
