Analyse des prescriptions d’inhibiteurs de la pompe à protons chez les personnes âgées en milieu hospitalier

Abstract : Background: proton pump inhibitors (PPI) are widely prescribed and particularly for elderly. Few data are available about conformity of these prescriptions for elderly (>65 years) in hospital. Objectives: to describe PPI indications and to identify the prescription associated factors in elderly hospitalized patients. Method: a transversal study of type "a given day" was realized in 24 care units of a teaching hospital. All patients more than 65 years were included. Results: on 436 patients included, 154 (35.3%) were treated by PPI. The most frequent reasons for prescriptions were treatment by antiagregant or anti-coagulant (68%). Prescription associated factors were the antecedent of gastroduodenal ulcer (ORa=6.66 ; IC95% [1.78-24.98]), or chronic heart failure (ORa=1.73 ; IC95% [1.03-2.91]), and previous treatment by low molecular weight heparin (ORa=2.96 ; IC95% [1.43-6.13]), not fractioned heparin (ORa=3.03 ; IC95% [1.56-5.90]), nonsteroidal anti-inflammatory (ORa=2.78 ; IC95% [1.09-7.09]) or corticoids (ORa=2.93 ; IC95% [1.26-6.80]). Conclusion: PPI are prescribed in one third of hospitalized elderly patients. Guidelines based on results of multivariate analysis will be work out to improve PPI prescriptions in elderly at hospital.
