Évaluation du dispositif Sport Sur Ordonnance de la ville de Saint-Paul à La Réunion : observation de l’évolution des critères morphologiques associés au risque de mortalité

Abstract : Background : in order to improve the health of its inhabitants, the city of Saint-Paul, in the Reunion island, suggests since 2014, a Prescription Sport Protocol (PSP) intended for sedentary adults affected by a chronic pathology : especially obesity, diabetes, arterial hypertension, osteoarticular diseases ... The main objective of this study was to observe the evolution of the correlation between a morphological index and the mortality risk (ABSI) after 9 months of participation in the protocol. Secondary goals of the study were to observe the evolution of the waist to height ratio, waist circumference, BMI and weight. Furthermore, the evolution of subjective symptoms were collected using Short Perceived Health Questionnaire (SPHQ) and pain evaluation for beneficiaries. Methods :
 judgment criteria were measured at the entrance and exit of the protocol. Results were compared by matched-group statistical analysis. Results :
 ninety-two patients were included in the protocol, including 80 (87%) women. A significant decrease was observed concerning the ABSI (-0.002 IC95 [-0.003; -0.001] p <0.0001), the waist-size to height ratio (-0.02 IC95 [-0.03; -0.01] p = 0.046) and the waist circumference (-2.6 IC95 [-4.1; -1.1] p = 0.0009). Scores for each SPHQ item increased significantly (+ 1.45 / 10 IC95 [1.15; 1.76] p<0.0001). The number of patients reporting pain, declined from 68 to 59 (-13.2%, p = 0.15).
Subgroups analyzes identified better outcomes for patients under 50 years old (mostly concerning morphological criteria), as well as those who entered the protocol only next to advices from their attending physician compared to those who asked their attending physician a sport prescription. Conclusion :
 our results suggest that the PSP allows the decrease of abdominal fat and mortality risk, mostly from cardiovascular origin. It also seems to improve the beneficiaries perceived health. Therefore, this device appears to be a relevant and effective intervention within public health.
