Coordination ville-hôpital en cancérologie : enquête d'opinion auprès des médecins généralistes prenant en charge des patients atteints de cancer colo-rectal suivis au CHU de Bordeaux

Abstract : Introduction: cancer is the leading cause of death in France. The general practitioner (GP) is a key player in coordinated care pathways, the “Hospital, patients, healthcare and territories” (HPST) act, and cancer Plans. This study evaluates the perception of the Gps and the patients about care coordination of patients with cancer. Method: a descriptive, cross-sectional study was performed between 10 March and 28 July 2016 with the 335 GPs named on the CPR forms of patients with colorectal cancer at Bordeaux University Hospital in 2014, and with the patients themselves. A questionnaire was distributed to the GPs by email and by post, and given to the patients at the day centre. Results: 25% of GPs responded. Most were male with a mean age of 53 years, and were following approximately 30 patients with cancer. The GPs were involved in the care of the patients, especially in screening (99%) and with the exception of therapeutic decisions (13%). 62% did not wish to take on a more substantial role. They were satisfied with the coordination among the healthcare establishments, although 90% were not familiar with the Oncology Communication Dossier (DCC), part of the Shared Medical Dossier, which according to the GPs does not improve care coordination. 25% knew the Regional Oncology Network, which they expected to provide better care coordination, compared with only 14% who knew the Oncology Coordination Centre (3C). They did not feel sufficiently informed by the Cancer Plans scheme (62%). Fifty-five patients responded, of which 58% thought their GP had a role in cancer care, but half did not wish GPs to be more involved and did not consult them regarding the issues of cancer. Conclusion: GPs and patients are generally satisfied with the colrectal cancer care coordination, this latter still to be developed.
