L’immunothérapie allergénique : vers de nouvelles approches thérapeutiques

Abstract : Allergen specific immunotherapy (AIT) is currently the only known treatment of allergic diseases IgE-dependent. This thesis explores a broad range of topics regarding AIT from mechanisms of action to the development of recent innovative treatments. In a first part, we give an up to date overview of immunological mechanisms of allergy, allergens, clinical symptoms and diagnosis of allergy. In a second part we propose a summary of current allergen immunotherapies: administration routes, indications, efficacy and security, production and reglementation, includinga focus on known AIT mechanisms of action. The balance between benefits and risks for AIT has not yet reached a perfect equilibrium. Most of development strategies aim to enhance immunogenicity (efficacy) and reduce allergenicity (toxicity) in order to make AIT more safe and effective. Thus, in a third part, we review these strategies based on the modification of allergenic extracts (recombinant allergens, hypoallergens, T- and B-cell-epitope-containing peptides), the use of adjuvants and the choice of new administration routes. We also focus on DNA vaccination for allergy treatment. AIT is a personalized medicine and would benefit from biomarkers that can predict AIT efficacy and give an early and a long term onset of AIT efficacy. Here we review the characteristics and the purpose of biomarkers, including an overview of biomarkers for AIT under development.
