L'analgésie péridurale : les déterminants de l'insatisfaction maternelle

Abstract : Objective : In 2014, in the level of maternity III of Auvergne region Hospital, 91.70% women gave birth with epidural analgesia. However, all patients were not fully satisfied about epidural and the suppressing of pain doesn’t seem to be the only element taken into account. The aim of this paper was to determine the predictive criteria of maternal dissatisfaction with epidural analgesia. Methods : An observational study of etiological cross-type mono centric was conducted in the level of maternity III of Auvergne Region Hospital. This report is part of AOMHE project led by the hospital anesthesia unit. The inclusions took place from 4 November 2013 to 24 April 2016. Results : A total of 147 women were included. Analyses involved 126 of them. This different predictive criteria of maternal dissatisfaction were identified with the multivariate analysis: multiparity, women with a BMI > 30kg / m², a high score to question 3 of the pre-partum, the presence of complications during labor, significant cervical dilation in the induction of analgesia. Conclusion : The setting up of a score and a predictive test of maternal dissatisfaction with epidural analgesia will enable to identify those women before and adjust their management (stronger analgesia, ambulatory analgesia, non-medicated relief methods...).
