Le bilan de médication en soins primaires : formation des pharmaciens à l’optimisation thérapeutique des patients âgés

Abstract : The increase in the prevalence of chronic diseases and the polypathological condition of the elderly population are the main factors explaining the increase in drugs consumption. In order to identify potentially inappropriate prescriptions, reduce iatrogenic events, and improve adherence to treatments, different actions may be proposed. Among them, a structured medication review (MR) can be provided by pharmacists in collaboration with general practitioner. The access to the medical history and the biological analysis of the patients implied by this new service will enable community pharmacists to carry out a level 3 pharmaceutical analysis. MR is supported by a professional Network for the Community Pharmacy (REIPO) structured around the city of Toulouse (Occitanie area, France). The main objective of this work here is to provide a status on the deployment of the communication campaign, as well as the training plan aimed at the pharmacists in the MR. The communication campaign has allowed to inform more than 300 pharmacists from Occitania. Among them, 77 pharmacists signed the membership charter for REIPO. The training kit developed by REIPO and its partners includes both remote training and onsite training on how to optimize the therapeutic treatment of the elder patients. Within this kit, the French Society of Clinical Pharmacy has created a blended-learning. It is currently being evaluated, and the initial feedback will help improving the next versions. REIPO pharmacists represent a real pool of resources that can be mobilized, and they can now benefit from a rich training offer that would allow them to take an active role as soon as medication reviews will start getting deployed in Occitania (BIMED'OC project). Therefore they will be able to adjust to the transition happening in their professional field more confidently.
