Contraception : évolution des pratiques des médecins généralistes dans le contexte de la polémique des pilules de 3ème et 4ème génération

Abstract : Oral contraceptive is one of the most efficient way of contraception and the most used by women in France. The oral contraceptive use increase the thrombotic risk. This risk is more increase with third and fourth generation pills than with second. This has been the subject of controversy in late 2012. The objective of our study was to evaluate the evolution of general practitioner’s practice about contraception and to evaluate the women opinion about third and fourth generation pills. 52 general practitioners took part to our study of 351 chosen randomly among general practitioners working in Hauts de Seine. 60% of general practitioners have said they practice gynecology and 40% have said they have additional training about contraception. All of the doctors were aware about the controversy. We observed a significant decrease (p < 0,001) of prescription before (50%) and after (23%) the controversy and a significant decrease (p < 0 ,001) of renewed prescription before (50%) and after (38%) the controversy. 50 women took part to our study, 52% used oral contraceptive. The opinion about third and fourth generation pills were split. 46% preferred another way of contraception, among them 22% preferred a second generation pill and 24% preferred a non hormonal method or natural method, whereas 46% didn’t have any opinion about this pills and 8% thought it was a good method. The controversy had long-lasting consequences on general practitioner’s prescriptions and on the choice of women’s contraceptives methods. However an improved gynecology’s training is necessary for few general practitioners.
