Comorbidité entre schizophrénie et trouble de stress post-traumatique, une revue de la littérature

Abstract : The prevalence of post-traumatic stress disorder (PTSD) is significant with 3.9% lifetime PTSD in the general French population (Lépine et al., 2005). It appears that individuals with severe psychiatric disorders, particularly schizophrenia spectrum disorders, are even more vulnerable to traumatic exposure and consequently to post-traumatic stress disorder. This work proposes a review of the existing literature on the association between schizophrenia and post-traumatic stress disorder. The first part deals with the definitions, the second is devoted to the methodology. The third part is the discussion of this review of the literature and is divided into three axes, namely, studies on the epidemiology of post-traumatic stress disorder in patients with schizophrenia, screening for PTSD in this area and the different treatments that could be used in common practice. An average prevalence of 12,4 to 30% of PTSD in patients with schizophrenia or schizophrenia spectrum disorder was thus found through this review of the literature. Included studies find an increase in depressive symptoms, suicidal behaviors, substance use disorders, secondary negative symptoms, and lower quality of life when patients present a PTSD. Results for positive symptoms and cognitive disorders are more contradictory. The Clinician Administered PTSD Scale is the reference diagnostic interview and four self-administered questionnaires are commonly used in these patients: the Impact of Event Scale, the PTSD Checklist, the PTSD Symptom Scale Self-Report and the Trauma Screening Questionnaire. For treatment, cognitive behavioral therapy, EMDR, post-exposure writing and some pharmacological treatments are likely to bring about an improvement in post-traumatic symptomatology.
