Aide aux Médecins généralistes lors de la rédaction du certificat d'aptitude à la pratique de la voile pour les plaisanciers à partir de l'étude de la base de données des accidents des Glénans

Abstract : Purpose: the practice of sailing presents inherent risks. A review of the literature reveals few articles on the subject. The purpose of this study is to is to establish reference points for the clinical exam required for obtaining a medical certificate for sailboat operators. Materials and Methods: this study is a retrospective review of case histories of accidents occurring at the Glénans school between 2009-2016. A total of 309 accidents were reviewed of 259 cases were analyzed for this study. Each case was analyzed according to the lesion location, trauma mechanism, trainee position and accident circumstances. We undertook univariate analysis for each component and invariant analysis for these components in relation to the type of boat Results: trauma was the most common lesion (96.91%) of which head trauma and loss of conscience the most frequent (28.19%) followed by skin lesions (25.87%) and joint trauma (25.1%). Head lesions are the most frequent (36.43%). Boat displacement (18.47%) and desalting (16.06%) carry the highest risks for accident. The boom is the most accident prone object (44.44%). Discussion : based on the results of our study we were able to define points of vigilance for the general practioner. Osteoarticular examination should seek joint instability or bone fragility. Cardiovascular examination should detect any symptoms of cardiac origin, an acquired or induced coagulation disorder. The neurological examination should look for disorders of balance or coordination of central origin. The psychiatric condition of the boater should be stable. He must ensure good oral health. Conclusion: this study has allowed us to establish a catalogue of accidents occurring at the sailing school of the Glénans. We were able to deduce the reference points for the physical exam required for obtaining a medical certificate and set up rules for prevention for sailboat operators. A model certificat based on these conclusions will be delivered to the headquarters of the Glénans.
