Prise en charge du diabète de type 1 par le médecin généraliste : compétences et rôle dans le parcours de soins. Une étude qualitative

Abstract : Epidemiologic data concerning diabetes mellitus type 1 highlights the slow but regular growth of this pathology. It is a major public health issue because it occurred quite early and because of its consequences. This qualitative study describes the management of diabetes mellitus type 1 by the general practitioner from his diagnosis to the follow-up. Ten semi-directing interviews from general practitioner in South-Finistère were retained. The data analysis was done according to the thematic method. From the diagnosis perspective general practitioner’s competences seem completely correct, even if the urgency of a ketoacidosis case seems underestimated. The use of urinary strip for the diagnosis won unanimous support, even if in certain cases a blood test could have been proposed. The immediate management is good enough with the catch of a systematic specialized opinion. The role in follow-up care seems to be limited to therapeutic education and tracking of the complications to the long term. Even if complications’ tracking of diabetes mellitus type 1 patient seem under control, therapeutic education is a vast and complex topic that would require a deepening of knowledge. Moreover, recent technological advances regarding the management of diabetes mellitus type 1 are barely known, which could be problematic in a near future in light of the increase of diabetes mellitus type 1 prevalence. The social and administrative role are mastered; the orientation of patients towards diabetic associations should be systematically proposed.
